Export-Only Medical Devices: Same Approval Number, Different Status |

Written and reviewed by Dr. Chihak Lee, medical director of Miso Clinic, Daegu.

The short version

An export-only medical device in Korea still carries a real MFDS approval number. The number looks exactly like a domestic one — same format, same "normal" item status. The only thing that separates them is one field buried in the lookup screen: "수출용에한함" (export only). A device approved that way is not approved for use on patients in Korea.

When we searched the MFDS registry for tissue-repair biomaterials (조직수복용생체재료, Class 4) on 4 September 2026, we counted 1,010 items — and 745 of them (73.8%) were export-only.

Why the gap exists

This is not somebody's mistake. It is how the rules are built. A Class 4 approval for domestic sale requires clinical data. An export-only approval does not carry the same burden — and the paperwork that gets skipped is precisely the paperwork that establishes safety and performance. For a manufacturer the cost of entry is not comparable.

745 items does not mean 745 products are circulating in Korean clinics. It means the domestic register holds that many export-only entries — which is exactly why a patient may want to check.

What the law says

Article 26(1) of the Medical Devices Act prohibits selling, leasing, granting or using a device that has not been approved. The only exception in the proviso is display at a fair or exhibition. Since an export-only product is not approved for domestic sale, using it here falls under "using an unapproved medical device." MFDS says the same in its own Q&A: an export-only device must obtain separate approval if domestic distribution is wanted.

The penalty figure we verified in the current text is ₩50,000,000. Statutes get amended, so if you are going to cite it, re-check the current article at the National Law Information Center rather than quoting this post.

How to check it yourself — 3 minutes, no account needed

  1. Ask the clinic for the approval number of the product
  2. Look it up at the MFDS medical device information portal (emedi.mfds.go.kr). No sign-up required.
  3. Open the detail view and confirm that "수출용에한함" reads 아니오.

Two traps worth knowing

"It doesn't come up" does not mean "it's illegal." We put well-known filler brand names into the name field and into the product-name field of the extended search, and they did not come up. The reason is mundane: the 제품명 field is blank for many entries — including the domestic product we examined. Search by approval number, never by brand name.

A lookup has a hard ceiling. It tells you whether that approval number may be used in Korea. It cannot tell you what is actually in the syringe in front of you. Korean-language labelling is another route, but if the clinic opens the outer box before you arrive you never see it. This is a real limit of the method, which is why we do not write "check it and relax."

One more thing clinics get wrong

Neither a clinic nor an individual may buy a medical device abroad and use it in Korea. Importing is reserved to licensed importers, and anything that arrives outside that channel is an unapproved device. Buying a machine you liked at a trade show and carrying it home is not allowed either — the exception in Article 26(1) covers display, nothing more.

Has anyone actually been caught?

We looked, and we could not find a public enforcement case of the specific type "a Korean clinic treated patients with an export-only device." We are telling you what we found and what we did not find, rather than implying either way.

What we did find: in March 2025 MFDS announced a monitoring system for illegal medical device distribution built with consumer groups and industry associations. The items named were laser hair removers, blood pressure monitors, cupping devices, anti-snoring devices, bruxism devices and nebulizers.
