Liquid vs. microsphere polycaprolactone (PCL): what's actually different?

🇺🇸 English

One-line answer: Microsphere PCL suspends smooth 25–50µm beads (30%) in a carrier gel, while liquid PCL is a solubilized, bead-free form (21% PCL). The human evidence differs greatly in depth: microsphere PCL has 1-year and 4-year biopsy data plus post-market data on 323,726 syringes, while liquid PCL currently has case-level data (2 patients, 12 weeks). No study has directly compared the two.

Same polymer, different products

Polycaprolactone (PCL) has been used in surgical sutures for decades. As an injectable, it comes in two very different forms. The microsphere type (the Ellansé family) suspends smooth beads of 25–50µm — 30% by volume — in a carboxymethylcellulose gel that absorbs in 6–8 weeks, leaving the beads behind. The liquid type (Gouri) has no beads at all: manufacturer materials describe 21% solubilized PCL with polymer chains below 0.1µm.

They're injected differently, too

Microsphere PCL is placed as limited boluses in the subcutaneous layer — supporting hollowed areas. Liquid PCL is spread across many shallow points in the deep dermis — addressing the skin surface over a wide area. Same polymer, different jobs.

The evidence is much thicker on one side

Microsphere PCL has actual human biopsy data: in 13 Asian women, dermal thickness was +26.7% at 1 year versus the untreated side, and +48.1% at 4 years in 3 patients (both P<0.001). There is also a 24-month randomized trial (40 patients) and post-market data covering 323,726 syringes with an adverse-event rate of 0.048% — one event per 2,089 syringes. For liquid PCL, the published human evidence is currently a 2-patient, 12-week case series, and we could not find any human biopsy data.

No head-to-head study exists

No study has directly compared liquid and microsphere PCL in humans — not split-face, not randomized, not in any form. So neither "liquid is better" nor "they're basically the same" has ever been tested. What you can compare is not outcomes but the depth of evidence.

Neither can be dissolved

PCL is a polyester, so hyaluronidase — the enzyme that reverses hyaluronic acid fillers — does nothing to it. That makes the initial placement and volume more important than with HA products. One recent report described 7 cases of bruising lasting months after liquid PCL in vessel-rich areas like the under-eye; all resolved completely with treatment.

FAQ

Q. Which one is better?

There is no basis to say — they have never been compared directly. Microsphere PCL has thicker data; liquid PCL is designed for treating skin over broad areas but its data is still accumulating.

Q. Does PCL really build collagen?

For the microsphere type, human biopsies show dermal thickening (+26.7% at 1 year, +48.1% at 4 years). Note that the Korean approval wording covers "temporary improvement through physical restoration" — collagen stimulation is not part of the approved indication.

Q. Can it be reversed if something goes wrong?

No. Neither form dissolves with hyaluronidase, so management relies on ultrasound localization and conservative treatment. That's why deciding the layer and amount before injecting matters more with PCL than with HA fillers.
