Does Retaining Ligament Filler Really Have Evidence Behind It?

🇺🇸 English

One-line answer: The most widely cited study behind retaining-ligament-targeted filler is a 45-patient case series with no control group, judged only by a 3-month global assessment; no randomized trial comparing it to conventional injection has been found, and the ligament anatomy the technique relies on is itself still debated.

What "retaining ligament filler" actually refers to

Retaining ligaments are fibrous structures that anchor skin and soft tissue to bone or deep fascia. The idea is that as these ligaments loosen with age, tissue folds downward along specific lines. Instead of filling hollows, this technique places small amounts above the loosened ligament to change the vector of pull. But "ligament filler" is a marketing term, not a standardized technique name — no confirmed standard protocol for injection points, volume, or depth exists.

The most-cited evidence is a 45-patient case series

The most widely cited report on this approach is Cohen et al., 2020, describing vector-based injection on the premise that laxity in the orbital, malar, and masseteric ligaments contributes to brow ptosis, nasolabial folds, and jowling respectively. But this is a 45-patient case series with no control group, judged only by 3-month global assessment — no randomization, no blinding. We could not find any randomized trial comparing this technique to conventional injection, or any trial using sagging as a primary endpoint.

The ligament anatomy itself is still debated

"Targeting the ligament" assumes the ligament's location and shape are settled. The tear trough ligament, described in a 2012 study of 48 cadaver hemifaces, became the standard picture. But a 2018 micro-CT study of 11 cadavers and 22 specimens suggested it may not be a separate ligament at all — possibly one continuous complex with the orbicularis retaining ligament. Histology studies questioning the existence of a true ligament are also cited secondhand. The ligament diagrams shown in consultations are a convenience model, not settled fact.

"It dissolves in a few months" doesn't always hold

The claim that small volumes are low-stakes because they resorb quickly doesn't match the under-eye data. A systematic review of 23 studies and 2,048 patients found 85% of HA volume still present at 15 months. A 155-patient study following patients an average of 785 days found improvement at 18 months was not statistically different from 6 months (p=0.57). We could not find equivalent data for the malar or masseteric ligament areas, but the "gone in 6-12 months" generalization did not hold in at least one area. That's why retreatment timing should be based on how things look, not a calendar.

"Small volume equals safe" doesn't hold for vascular events

Vascular events are determined by whether a single injection enters a vessel, not by total volume — 0.1mL into an artery is already an accident. A review of blindness cases found 48 new cases between January 2015 and September 2018: 27 in the nose (56.3%), 13 glabella, 9 forehead, but also 7 nasolabial and 2 cheek cases. The midface isn't the most common site, but cases are documented there.

It's reversible, but products vary a lot

HA filler is reversible with hyaluronidase, but how well it dissolves varies widely by product — up to a 12.5-fold difference in what we compiled. Non-HA materials don't dissolve this way, so confirming what's actually being injected matters. In Korea, HA fillers are approved as medical devices, and we could not confirm any product approved with "ligament lifting" or "sagging improvement" as its stated intended use.

How Miso Clinic approaches this

This technique is neither banned nor validated — it's a low-evidence option. When we discuss the ligament-targeted approach, we mention that the supporting data is a 45-patient case series. We favor reversible material and discuss it upfront, and we don't schedule retreatment by the calendar; we decide based on how the area looks.

FAQ

Q. Does ligament-targeted filler really lift the face? Some reports say so, but the evidence grade is low. The most-cited study is a 45-patient case series with no control group, judged only at 3 months. No randomized comparison to conventional injection has been found.

Q. Isn't a small volume automatically safer? Total volume and vascular events are different issues. Even 0.1mL entering an artery is an accident. In a blindness case review, 7 of 48 new cases were nasolabial and 2 were cheek — midface cases.

Q. What if I don't like the result — can it just be dissolved? HA filler can be reversed with hyaluronidase, but products vary up to 12.5-fold in how well they dissolve, and non-HA materials won't dissolve this way at all.
