제형이 얼마나 갈라놓나요
2025년에 나온 반얼굴 비교시험이 이 질문을 직접 다뤘어요. 같은 사람 얼굴 한쪽엔 기존 PN 제형, 반대쪽엔 pH와 이온강도를 조정한 제형을 넣었어요.
- 엠보싱: 기존 제형 56.7%, 조정 제형 13.3%
- 홍반: 60% 대 40%
- 주입 통증(VAS 10점): 5.04 대 3.39
- 주름 개선: 두 제형 사이에 유의한 차이 없음
여기서 읽을 건 "제형을 바꾸면 구진과 통증이 줄 수 있다"예요. "효과가 더 좋아진다"는 아니에요. 그리고 15명짜리 시험이라 비율을 그대로 믿진 않아요. 저는 방향을 참고하는 정도로 봐요.
언제까지가 정상인가요
- 시술 당일: 구진과 붉은 기가 있어요. 이때 얼굴은 최종 결과가 아니에요. 사진으로 판단하지 마세요.
- 1~2일: 구진 대부분이 흡수돼요.
- 며칠: 바늘이 작은 혈관을 지났다면 멍이 남을 수 있어요.
일주일을 넘겨 남아 있거나, 가라앉았다가 몇 주 뒤 새로 만져지거나, 통증과 열감과 점점 심해지는 붉은 기가 있으면 연락 주세요. 피부색이 창백해지거나 그물 모양으로 변하면 바로 연락하셔야 해요. 드물지만 놓치면 안 되는 신호예요.
결절과는 뭐가 다른가요
구진과 결절은 자주 섞여 쓰이지만 기전이 달라요. 리쥬란의 구진은 액체 부피가 만든 융기라 흡수되면 사라져요. 입자형 제제의 결절은 남아 있는 입자에 조직이 반응해서 생기고, 문헌상 지연성 발생은 시술 후 3~4개월쯤이 가장 많아요. 리쥬란은 분해형이라 이 기전이 해당하지 않아요.
줄이려면 어떻게 하나요
근거가 있는 건 제형 선택 하나예요. 주입 깊이와 한 점당 양을 조절하는 것도 기전상 맞지만, 사람에게서 무작위로 비교한 시험은 찾지 못했어요. 시술 직후 냉찜질이나 당일 사우나·음주 피하기도 널리 안내되지만, 구진을 줄인다는 비교 자료는 확인하지 못했어요.

One-line answer: Bumps right after Rejuran (papules, or "embossing") are the injected liquid sitting in the dermis for a short while. They are an expected response, not a failed treatment, and in the literature they usually settle within 1 to 2 days. How many appear depends a lot on the formulation.
Why do the bumps show up at all?
At Miso Clinic in Daegu, Rejuran goes into the dermis in small amounts at many points, spaced about 3 to 5 mm apart. The liquid at each point doesn't spread out right away. It stays put briefly, so each point becomes a tiny hill. What you see is the volume of the product, not an irritated tissue reaction.
Size and count come down to dose, depth, spacing and formulation. Shallower placement and more product per point make bumps more obvious. Thin skin around the eyes shows them more. Same reason.
How much does formulation matter?
A 2025 randomized split-face study of 15 people looked at exactly this. One side of each face got a conventional PN product, the other side got a version with adjusted pH and ionic strength.
- Embossing: 56.7% on the conventional side vs 13.3% on the adjusted side
- Redness: 60% vs 40%
- Injection pain (VAS out of 10): 5.04 vs 3.39
- Wrinkle improvement: no significant difference between the two
So what the study shows is that changing the formulation can reduce bumps and pain. It does not show better results. And it is 15 people, so I would not generalize those percentages. I treat them as a direction, nothing more.
What is normal, and until when?
- Same day: bumps and some redness. Your face at this point is not the result. Please don't judge it from a photo.
- Day 1 to 2: most bumps get absorbed.
- A few days: a bruise can linger if the needle passed a small vessel.
Contact the clinic if a bump is still there after a week, if something new appears weeks later, or if you get pain, warmth or spreading redness. Pale or net-like skin color is rare, but call right away.
How is this different from a nodule?
People mix up "papule" and "nodule", but the mechanism is different. A Rejuran papule is a bump made by liquid volume, and it disappears as the liquid is absorbed. A nodule from a particle-type filler comes from tissue reacting to particles that remain, and in the literature delayed onset peaks around 3 to 4 months after treatment. Rejuran is a degradable product, so that mechanism doesn't apply.
Can you reduce bumps?
One thing has direct evidence: formulation choice. Adjusting depth and the amount per point makes sense mechanically, but I couldn't find a randomized human trial comparing them. Cold compresses and avoiding saunas, alcohol and hard workouts on the day are widely advised, yet I found no comparison data showing they lower papule rates.
So my advice doesn't depend on evidence. Don't book Rejuran two or three days before something important. I say this first in the consultation room at Miso Clinic.

Q. Are the bumps on the day a side effect? Usually not. They are a physical raised area from the liquid pooling in the dermis, and they typically settle in a day or two.
