Why Does Botox Seem Not to Work, and Is It Really Resistance? |

One-line answer: True Botox resistance from antibodies is uncommon. When it seems like a shot didn't work, I check four things first: how many days you waited, whether the line is a movement wrinkle, which product and units were used, and whether the injection reached the right muscle. At Miso Clinic in Daegu, we space the same area at least 3 months apart.
"It didn't work. Is it resistance?"
I hear this in the consultation room all the time: "nothing happened this time" or "I think I built a tolerance." Antibodies can reduce the effect, and that does happen. But in my experience, four simpler explanations come up before that one.
Did you judge it too early?
Botox starts working one to two days after the injection and gets stronger through the first week. In a randomized trial of 180 people, the median onset was 5.3 days. So looking in the mirror on day two or three and calling it a failure is premature. We check at two weeks, and if one spot responded less, we touch it up with a small amount.
Is the line there even when your face is still?
Botox quiets muscle movement. It suits lines that appear when you frown or smile. A line that has been folded for years and is etched into the skin won't flatten right away just because the muscle moves less. That isn't Botox failing. It's a job Botox wasn't built for.
Which product, how many units, which muscle
Units measure potency, not volume, so the same number from a different product is not the same strength. If you remember the product name, the units, and the date of your last session, my judgment gets much easier. When only one side responds less, or one patch keeps moving, the muscle there was usually under-covered. That's a design problem, not resistance, and we fix it at the two-week check.
When I do think about resistance
The pattern is a treatment that worked well at first, then weaker each time, then barely at all. The body may have made neutralizing antibodies. A review lists short intervals between top-up shots, large amounts per session, and more antigenic protein as related factors (Bellows and Jankovic 2019). I'll be honest: we could not find how common this is at cosmetic doses. So I don't answer "it didn't work" with "let's add more." We keep at least three months between sessions in the same area.
People also ask whether switching products brings the effect back. There is one case report, and some of its authors work for the manufacturer. That's too thin to call a general fix.
When I don't recommend Botox
- Under two weeks since the injection and you want more because it feels weak
- Less than three months since the last session in the same area
- Deeply etched lines at rest are your main concern
- Pregnancy, breastfeeding, or a neuromuscular disease such as myasthenia gravis
If your eyelids droop on both sides, you see double, swallowing or speaking gets hard, or you feel short of breath after an injection, go to an emergency room right away.
FAQ
Q. If Botox doesn't work, is it resistance?
Usually not. Check timing, wrinkle type, product and units, and muscle targeting first. Antibody resistance is uncommon.
Q. When should I judge the result?
Onset begins within one to two days and builds over the first week. We check at two weeks.
Q. How far apart should Botox sessions be?
We space the same area at least three months apart, and we don't add more within days just because it feels weak.
