출처를 모르는 톡신, 실제로 사고가 났어요
미국 질병통제예방센터(CDC) 주간보고는 2024년 3월 테네시와 뉴욕시에서 비의료 장소에서 위조로 추정되는 톡신을 맞은 뒤 생긴 증상들을 보고했어요(Thomas 2024). 중국의 한 병원은 10년 동안 미용 주사 뒤 보툴리눔 중독 161건을 정리했는데, 대부분 출처를 알 수 없는 톡신을 무면허 시술자에게 비의료 장소에서 맞은 경우였고 중등도 이상이 최대 60%였어요(Qiu 2025).
주사 뒤 눈꺼풀이 양쪽으로 처지거나, 물건이 둘로 보이거나, 삼키기·말하기가 어렵거나, 숨이 차면 바로 응급실로 가세요. 날짜를 세지 마시고요.
정품 확인, 어렵지 않아요
상자와 라벨을 보여 달라고 하세요. 제품명, 제조번호, 사용기한이 적혀 있어요. 그리고 그 자리에서 개봉하는지 보세요. 미리 꺼내 두거나 주사기에 담아 온 것은 무엇인지 알 수 없어요. 보툴리눔 톡신은 의약품이라 식약처 의약품안전나라에서 제품명으로 허가를 찾아볼 수 있고, 리쥬란 같은 의료기기는 품목허가번호로 조회해요. 병원 안내물의 긴 번호는 광고 사전심의번호인 경우가 많은데, 허가번호와는 다른 번호예요.

One-line answer: Phrases like "how many units" or "how many cc" tell you very little about what is actually injected. Ask to see the box and label (product name, lot number, expiry), watch it being opened in front of you, and ask about the amount of drug, not the amount of saline it was mixed with. Start with the phrase "we give you a lot"
Botulinum toxin comes as a powder that gets dissolved in saline. The amount of drug, measured in units, stays the same no matter how much saline is used. One hundred units dissolved in 2.5 mL is still 100 units in 5 mL. So when a clinic says "we inject a lot," you have to ask: a lot of drug, or a lot of water? Those are different things.
More water means more volume at each injection point. One study found that raising the volume per point from 0.025 mL to 0.1 mL, with the same units, roughly made a spread index 2.2 times larger outside the target muscle (Rahman 2025). To be fair, that was a computer simulation, not a measurement in people. Read it as a direction, not a precise number.
Toxin of unknown origin has caused real harm
A CDC weekly report described people in Tennessee and New York City who developed botulism-like symptoms in March 2024 after injections of suspected counterfeit toxin in non-medical settings (Thomas 2024). A hospital in China collected 161 cases of botulism after cosmetic injections over ten years; most involved toxin of unknown origin, injected by unlicensed providers outside medical facilities, and up to 60% of cases were moderate or worse (Qiu 2025).
If your eyelids droop on both sides after an injection, if you see double, have trouble swallowing or speaking, or feel short of breath, go to an emergency room. Do not wait it out.
Checking the product is not complicated
Ask to see the box and the label: product name, lot number, expiry date. Then watch whether it is opened in front of you. A pre-filled syringe brought from another room could contain anything. In Korea, botulinum toxin is a licensed drug you can look up by name in the regulator's database, and devices like Rejuran have a product license number. The long number on a clinic's poster is often an advertising review number, which is a different thing.

Does pre-mixed toxin lose its effect?
Not necessarily, if storage rules were followed. A double-blind trial injected toxin that had been reconstituted up to six weeks earlier, stored per the manufacturer's instructions, into the glabella of 85 analyzed patients. The effect did not differ by how long ago it was mixed (Hexsel 2003). So the real question is not "was it pre-mixed" but whether the clinic can tell you what was mixed, in how much, and how it was stored.
Same unit number, same dose? Not across products
Units measure biological potency, not volume, and each manufacturer uses its own potency assay. Units of one brand do not convert one-to-one into units of another. A unit count only means something together with the product name.
Rejuran's "cc" has the same problem
With Rejuran, "cc" usually refers to syringe size. What to check is how many syringes are actually opened for you, and whether anything else is mixed in. Two cc from two products is not the same procedure as one product stretched with another liquid. I have not found a human study comparing diluted versus undiluted PN, so I will not claim dilution is fine, or that it ruins the result. What I can do is show you exactly what is opened and used.

- For toxin: how many units, dissolved in how many mL?
- For Rejuran: how many syringes will you open, and is anything mixed in?
- How do you check the result at two weeks?

Q. How do I check that a botulinum toxin is genuine? Look at the box and label for the product name, lot number, and expiry date, and watch it being opened on site. In Korea you can look up the license by product name in the drug regulator's database.
Q. Does diluting Botox weaken it? Dilution changes the water, not the units. But injecting the same units in a larger volume may spread further beyond the target, according to a computer-model study. It was not measured in people.
