Nasolabial Folds in Daegu: Which Treatment Fits, and Why Does the Cause Come First?

🇺🇸 English
One-line answer: A nasolabial fold is not a skin crease. It is a structural boundary line created by facial muscles attaching directly to the dermis, and three overlapping causes feed it. That is why "which treatment is right?" has no single answer, and why the facial artery running through this area is discussed first.
Three causes behind one line
In the mirror it looks like one line, but three things build it.
- Muscle attachment to the dermis: muscles that lift the upper lip attach directly to the dermis (the thick middle layer of skin), pulling the skin along that line every time you smile. Filling does not remove the attachment itself.
- Descent of the cheek fat compartment: the upper fat pad slips down, so the area above the line looks fuller and the area below looks more hollow.
- Bone resorption of the upper jaw: the bone recedes, and the soft tissue above it loses support.
The share of each cause differs from person to person, so the treatment that fits can differ even when the fold looks the same.
What is not the answer
- "Filling makes it disappear": if muscle attachment is the main cause, the line remains after filling. Adding more volume can make the smile look unnatural.
- "A collagen booster removes it": boosters target dermal density and texture. Nasolabial folds are a structural issue, so making a booster the primary goal points in the wrong direction. That does not mean boosters are meaningless.
- "Lifting smooths it out": radiofrequency and focused ultrasound target sagging. That fits when cheek fat descent is the main cause, but may fall short of expectations when bone loss or muscle attachment dominates.
The risk we mention first
The facial artery runs through this area. In data on vision loss across filler treatments overall, 365 new cases showed full recovery in 6.0% and no recovery in 68.2% (Doyon VC et al., PMID 38630871). This number is not specific to nasolabial folds, but it belongs next to any decision about this site.
The evidence differs by approach
- Fillers: a relatively thick body of data for restoring volume.
- Threads: a thinner level of evidence than fillers, and hard to reverse.
- Focused ultrasound (HIFU, which creates small heat points at set depths): aims at the sagging above the fold rather than the fold itself.
- Radiofrequency (RF): a 2026 systematic review covered 15 studies and 1,230 people, with only one randomized controlled trial, and no head-to-head comparison between devices was identified.
That combining approaches works better was not confirmed in two randomized trials. The reason to consider a combination is that different layers are being treated.
How the consultation goes
1. We look at you smiling and at rest. 2. We check the upper cheek for descent. 3. We check what material has already been placed. 4. We explain the vascular risk before any decision. 5. We do not do everything at once, and we split the steps in order.
The plan follows your face as it is now, not a product name. If the cause narrows to one, only one approach is used. Not recommending something that isn't needed is part of good care.
FAQ
Q. Is filler the right choice for nasolabial folds? It depends on the cause. If bone loss or volume loss is the main driver, the direction fits. If muscle attachment creates the line, the line stays even after filling. The facial artery risk is weighed together with this.
Q. Can thread lifting raise nasolabial folds? The direction fits when cheek fat descent is the main cause. But the evidence for threads is thinner than for fillers, and they are hard to reverse.
Q. When can I judge the result? It differs by approach. Filler is at its maximum right after treatment and then declines. In a rater-blinded study of 20 Korean women, radiofrequency was not significant by clinician assessment at 4 weeks, was significant at 12 weeks, and peaked at 4 to 6 months.
