Why skin thickness and fat should change how a lifting treatment is planned |

🇺🇸 English
One-line answer: Facial skin thickness varies mildly (about 1.5–2.0mm between regions), but the superficial fat beneath it varies more than threefold — 1.6mm to 5.1mm. HIFU cartridges come in fixed 1.5mm depth steps, so natural anatomical variation is larger than the device's adjustment unit. That's why depth, power, and tip size should be varied by region and by person.
The face is not one uniform sheet of skin
A cadaver study measuring 53 faces region by region found skin thickness ranged only from 1.51mm (nose) to 1.97mm (under-eye) — but superficial fat ranged from 1.61mm to 5.14mm around the mouth, a 3.2-fold spread. Use the same cartridge on the cheekbone and around the mouth, and the energy lands in anatomically different places.
Ultrasound focal depths are fixed
HIFU cartridges are built to fixed depths — 1.5, 3.0, 4.5mm. In phantom and cadaver work, the 4.5mm coagulation points landed from superficial fat through the SMAS depending on region, the 3.0mm cartridge also produced coagulation in the dermis in front of the focus, and the same labeled depth produced different lesion shapes across devices. "4.5mm" doesn't mean the same thing between brands.
Radiofrequency heats different tissues differently
Around 1MHz, dermis conducts at roughly 0.25S/m and subcutaneous fat at 0.03S/m — nearly an 8-fold difference — so the same power produces different temperatures depending on fat thickness. And in a 31-person split-face randomized trial, the smaller 3cm² tip was significantly better around the eyes (p<0.001): shallow areas do better with electrodes that reach shallow.
Thickness doesn't promise results
There's data in the opposite direction, too. In a 25-patient study followed for 12 months, neither skin thickness nor fat depth predicted treatment response — the only significant predictor was tissue mobility (less sagging responded better). Thickness informs how to treat (design); degree of sagging informs how much to expect (expectations). Different questions.
The real reason depth matters: safety
A 45-study review of HIFU adverse events found facial fat atrophy in under 1% of cases — rare, but attributed to "inappropriate depth selection." Not using deep cartridges on thin-fat areas — temples, around the eyes, hollowed cheeks — is the clearest practical payoff of paying attention to thickness.
How we apply this
We divide the face into regions, use shallower depths and smaller electrodes on thin areas, assess sagging separately to set expectations honestly, and distinguish whether the problem is texture and density (booster territory) or sagging (lifting territory).
FAQ
Q. Does measuring thickness with ultrasound before treatment improve results?
The direction is supportive but unproven: a 20-person study favored the anatomy-based approach, but differences were sub-millimeter. The clearest benefit of thickness assessment is safety, not efficacy.
Q. Does thick skin respond worse to lifting?
Thickness itself didn't predict response in the available data — sagging did. Thickness is a design variable for choosing depth, tip, and power.
Q. Can BMI stand in for facial thickness?
Multiple studies report BMI can't predict facial skin thickness. Body fat and facial fat move independently, so the face has to be assessed on the face.
